APS Funding Application
Fill out all sections of the funding form.
See page 6 for all required documents that must be attached to the application.
Incomplete applications will not be considered or reviewed.
If you require assistance filling out the application, please contact the APS manager or APS assistant to set up an appointment
Reimbursement Form
Reimbursements are available for sponsored students ONLY. Please contact APS Admin team to inquire about what is reimbursable.

Reimbursements must be submitted to APS admis team by 4 p.m. Tuesday of every week.
​
Receipts are required for reimbursements; Payments will not be processed without proper receipt(s).
Progress Report Form
Progress Report(s) are Due on the 15th of every month.
​
All NEW students or students placed on academic probation are required to fill out PR for ALL sponsored classes.
